Why partner with us

People and their bodies are complex, and no two people’s healthcare needs are the same.

We are a small practice of GPs who specialise in personalised chronic disease management and health promotion.

The TKYC was established by Dr Laura Chapman, an experienced GP and chronic disease sufferer herself. After years of working in different GP practices, Laura came to appreciate that complex, chronic health conditions require time, and a deep understanding of the individual.

Due to various, intersecting factors, including long-term freezes on Medicare rebates, General Practice has evolved over time, by necessity. Practices often adopt a high turnover model of care, in order to stay financially viable. This high-volume model can keep per-appointment costs down for individual patients, but with potential, personal pay-offs:

  • Feeling rushed.
  • Feeling as though care is not individualised to specific needs
  • Feeling unheard.
  • Feeling like there is a limit of one issue per appointment, increasing the number of total appointments needed.

The Know You Co loves to foster relationships. This includes investing time in YOU, as well as with other care providers. By nurturing our relationships, we work in partnership with allied health providers and non-GP specialists to help achieve the best, co-ordinated health outcomes for our patients.

We will always keep your treatment team up-to-date, and depending on your need, we may co-ordinate formal multidisciplinary meetings. Our aim is to provide and oversee cohesive care, like the project managers of your healthcare. So you can outsource to us…we love it!

What is Health Promotion?

Health promotion is the act of minimising harm and optimising wellbeing. This includes screening, check-ups and developmental checks.

Health promotion might look different for an individual with a chronic health condition, or perhaps for a person with a complex family history. For example, your bowel cancer screening requirement might look different if there is a family history, or your immunisation recommendations might be adjusted if you have asthma.

What is a Chronic Health Condition?

A chronic condition by definition is one that has been present for some time, or is likely to persist. A chronic health condition can be present, but well-managed. Examples can include mental illness, autoimmune disease, hypertension, heart disease, kidney disease, women’s health conditions (endometriosis, PCOS etc), diabetes, oesteoarthritis…the list goes on!

Managing chronic illness requires time and an understanding ofan individual’s specific needs. Similar to “corporate knowledge”, this cumulates over time when a clinician has seen you throughout the ebbs and flows of your illness.

How

Getting to know you

First appointment, 1 hour

Your first appointment will be all about getting to know YOU!

This will involve taking a deep-dive into your medical history and clarifying your goals of care We will always check to ensure your medications and referrals are up to date.

At the end of your initial appointment, a schedule of follow-up appointments will be discussed and booked – this means you can get on with your life and not have to scramble to make appointments at the last minute.

Fee: $427

Medicare Rebate: $197.60

Out of Pocket: $229

Know your stuff

Results appointments

You will always hear from us with your results. Sometimes this will be as simple as receiving a text message from us, confirming that everything is a-o-kay, or if we need to make minor changes to your action plans.

If your results require a bit more discussion, we will offer you a 15-minute Telehealth with your doctor. This will generally occur within a week.

Fee: $112

Medicare Rebate: $42.85

Out of Pocket: $69

In the know

Follow-up appointments, 30 minutes

Follow-up appointments will be booked for typically 30 minutes. Longer follow-up appointments can be arranged if needed.

We will tailor a follow-up regimen specific to your specific needs, and may be booked for every 6 months, or more frequently if needed.

Urgent appointments

The thing about chronic conditions is that they don’t always follow a predictable pathway.

If you have a need to be seen more urgently than your next booked appointment, just get in touch with us. If needed, we aim to be able to see you within the week, according to urgency.

We don’t reliably have available appointments “on the day”, but if in doubt, just reach out to us and we can provide you with some advice.

Appointment housekeeping

You’re busy, we get it. We aim to run on time for all appointments. This means we will keep an eye on the clock and let you know when your appointment end-time is drawing near. Please don’t take this personally, we’d spend all day with you if we could! But clock-watching ensures that we run to time, and you don’t have to spend hours sitting and waiting.

If you’re running late for your appointment, don’t stress. We’ve all been there. BUT we will need to finish at your designated finish time nevertheless.

Sometimes life throws you a curve ball and you need to cancel your appointment, right? We appreciate 24-hour notice where possible, and otherwise you may incur a cancellation fee if you cancel with less than 4-hours notice.

Laura started her career in communications. Her love of human connections and relationship building led her to pursue a career in medicine. Laura is a chronic disease sufferer herself, having been diagnosed with inflammatory arthritis in her first year of medical school. As such, Laura has developed a passion for providing support and care for people whose complex and ongoing care needs seem to fall outside of the typical model of general practice.

Laura loves working with her patients to develop individualised plans. She loves a challenge and, while she is well aware of the limitations to her knowledge, she enjoys learning and growing as a practitioner, alongside her patients.

Megan Phipps & Charlotte Rooks

Practice Managers

Stuck together like in a three-legged race, Charlotte and Megan are TKYC’s Co Practice Managers. Both having spent years in the medical administration space, they know the ins-and-outs of the system and how to support a Practice to provide patients with the best healthcare experience possible!  As fellow chronic disease-goers themselves, Charlotte and Megan are passionate about TKYC’s mission and understand the importance of chronic disease management and health promotion. The next time you swing past the Co, make sure you ask Charlotte to show you pictures of her dazzling canine companion, and give Megan your recommendation on Canberra’s best laksa!

Charlotte holds a Bachelor of Business (Human Resource Management) and is studying a Master of Occupational Therapy. Megan is a member of the Australian Association of Practice Management (MAAPM), and is about to graduate with a Bachelor of International Security Studies, and a Bachelor of Arts (Linguistics). She has plans to undertake further studies in a Master of Speech Pathology.

Dr Jung Le-Qui

Dr Jung Le-Qui’s relationship with TKYC founder spans over 15 years, as Laura’s steadfast GP. Jung has stood by Laura’s side through many of life’s highs and lows, and has had a huge role in shaping Laura’s own style of practice.

Having graduated from Queensland University in the early 1990s, Jung has dedicated over three decades to serving the Canberra community. Yet, her personal experience with metabolic challenges, particularly hyperinsulinemia and associated hypoglycemia, has been her greatest guide. Jung has navigated her own metabolic variant amidst outdated health recommendations, championing a holistic understanding of metabolic disorders and weight-related chronic health issues.

Jung’s love for metabolic health can be summarised by peas…we mean PsPassion for delivering healthcare to those with shared metabolic challenges, patience in supporting patients to feel heard, and a commitment to a personalised, patient-centered care. Jung loves working in collaborative, multi-disciplinary teams, and empowering her patients to reclaim control of their health journey.

Dr Krystle Marca

BMedSci, MBBS, DCH, FRACGP

With over 10 years’ experience as a GP, Krystle is passionate in providing excellence in her clinical care and establishing continuity in relationships. Understanding that every patient is unique, Krystle takes a whole-person approach to each and every consultation, and enjoys unpacking a patient’s history and healthcare goals. Worn out by high throughput medicine, she is excited to be joining TKYC and positively contributing to its vision of high quality, personalised, and collaborative health care.

Her strengths are in women’s and children’s health, with further qualifications in these areas. Examples include contraception, PCOS, endometriosis, sexual health, menopause, thyroid disorders, preventative health, pregnancy care, child development, asthma, eczema, depression, and anxiety.

Krystle is a loving wife and devoted mum of four. Outside of work she enjoys a good cup of coffee, walking her dogs, exploring the outdoors with her family, eating great food, and slowing down to appreciate all that’s around her.

Joni

Laura’s puppy!

Joni is our aspiring support dog (for us as well as our patients!). She has an interest in sniffing and snacks, and is trying really hard not to bark at random, irregular intervals when something feels not quite right in her world. Joni is hypoallergenic and very on board with cuddles for all. She is still growing her confidence in the workplace, and you will often find her following Laura like a shadow.

FAQ

Q: What are your fees?

Initial, Getting to Know You appointment – $427 per hour (with a Medicare rebate of $197.90)

Follow-up “in the Know” appointments – $223 per 30 minutes (with a Medicare rebate of $82.90)

Phone appointments – $112 per 15 minutes (with a Medicare rebate of $42.85)

Script and referral requests – we may be able to provide you with scripts and referrals outside of appointments. This incurs a fee of $10 for scripts, and $20 for referrals.

Q: What if I need a more urgent appointment?

We are not an acute service. We arrange our appointment books with some time each week ruled off for urgent appointments. This means that, according to your need, we can usually see you within a week. We are unable to reliably offer “on the day” appointments, but can help direct you to other options if needed. Don’t hesitate to reach out to us as the first port of call.

Q: Can I expect to hear from you about my test results?

Yes. Always! You can expect to receive notification from us within a week of receiving your results. This may be a simple text message, letting you know that everything is fine. If something comes up that requires more discussion, we will be in touch to offer you a telehealth appointment, or an urgent face­-to-face appointment if needed.

Q: What services do you provide at TKYC?

We provide medical care. We are able to help you set up referrals to other healthcare providers, Mental Health Care Plans, Eating Disorder Plans, and Chronic Disease Management Plans.

We are passionate about the provision of coordinated, multidisciplinary care. Sometimes we might talk to you about hosting a multidisciplinary meeting with your other providers present.

We have limited availability for procedural work, but please don’t hesitate to reach out to our team for more information.

Unfortunately, we are unable to provide childhood immunisations, COVID or Flu vaccinations, or other immunisations on the National Immunisation Program. We work closely with local immunisation providers, and can recommend alternative avenues. Just give us a ring, or flick us an email!

Q: Your books are closed, what do I do if I want to become a new patient?

Our books are temporarily closed. By filling out the self referral form found on our website under ‘New Patient’, you will be placed on a waitlist and our team will reach out when an appointment is available.

We only take on the patient numbers that we can manage. This keeps us available to our existing patients. Every so often, we will temporarily close our books while we reassess whether we’re in a position to take on new patients, and provide them with the ongoing support that they need.

Q: Where are you located?

We are located in the Peter Yorke Building on Level 3, 173 Strickland Crescent in Deakin, ACT.
There’s street parking, parking on the Calvary John James Campus, and parking attached to the Deakin Bowling Club just nearby.
